I am a young person with additional needs

Support for young people with additional needsIf you have additional needs you have the chance to talk with a youth support worker about the support you need as part of your transition review - 'thinking about my future'

Open the transition card now

You can use this card with a youth support worker, parents or carers to think about your future and what support you need.

Transition review

The card can also be used in your transition review in school or college. (A youth support worker will bring a copy of the card too).

In the review you can start to plan the support you will need and record it in a action plan

Your parents or carers can attend the transition review as well.

You can also use the card at any other time when you are moving on to something new. For example leaving school.
